Saphnelo is a disease-controlling treatment, not a symptom reliever: it reduces lupus activity and flares and can help lower steroid doses, but the benefit builds gradually — judge it over 3–6 months, not days. It works alongside your other lupus medicines, which should be continued unless your provider changes them.

What it is

Saphnelo (anifrolumab) is a biologic medicine for adults with moderate-to-severe systemic lupus erythematosus (SLE) who are already receiving standard lupus treatment. It blocks the receptor for type I interferon — a messenger protein that is overactive in many people with lupus and acts like an alarm signal that keeps the immune system switched on. Calming this pathway can reduce lupus flares (times when the disease becomes more active) and may allow some patients to lower their steroid dose. It is added to — not a replacement for — your other lupus medicines.

Before starting

Before starting, we review your infection history and vaccination status. Shingles (herpes zoster) occurs more often on this medicine, because interferon is part of the body’s defence against that virus — so we may discuss the shingles vaccine before you begin, since vaccines work best when given ahead of treatment. Live vaccines should be avoided during therapy. Saphnelo is not used for severe, active lupus of the kidneys or the nervous system, which need different treatment approaches. Tell us about any past serious or recurring infections, and about any medicines you take, so the plan fits your full picture.

How it’s given

It is given as an IV infusion — medicine dripped slowly into a vein — in our infusion suite, once every 4 weeks. The infusion itself takes about 30 minutes, and the whole visit usually about an hour. There is nothing to inject or store at home. The clinical team checks on you during the infusion, and you can go back to your normal activities afterwards.

What to expect

Saphnelo’s benefits build gradually over the first few months: fewer flares, easing of lupus skin rashes and joint pain, and for many patients, the ability to taper steroids down. Results are usually judged over 3–6 months rather than days. The most common side effects are cold symptoms and other upper respiratory infections, bronchitis, cough, and infusion-related reactions such as headache or feeling unwell during the drip, which the team can usually manage by slowing the infusion. Because immune defence is altered, call us promptly for fever or an infection that is not improving — and let us know quickly about a painful blistering rash, which could be shingles.

Everyday guidance

Keep your monthly infusions on schedule: steady treatment is what holds flares down, and gaps let lupus activity return. Continue your other lupus medicines unless your provider changes them, and do not reduce steroids faster than the taper plan you are given. If you are ill with an infection near an infusion date, call us first — we may move the appointment until you recover. Tell other clinicians you receive an immune-calming infusion, and check with us before vaccines or surgery. Questions day or night: (361) 400-7700.

Common worries

Why monthly infusions if I already take lupus pills?

Saphnelo blocks a pathway (type I interferon) that the pills do not target. Lupus often needs more than one angle of attack, and this one is aimed at the alarm signal that keeps lupus switched on.

I’ve heard shingles is more common — true?

Yes, shingles occurs somewhat more often on Saphnelo, because interferon helps keep that virus in check. That is why we discuss the shingles vaccine before starting, and why a painful blistering rash should be reported to us the same day — early treatment helps.

Will it replace my steroids?

Not directly — but by controlling lupus activity it often lets us taper steroids down over time, which is one of its most valued benefits.
