New patients & referrals
Your first visit
Your first appointment runs longer than a typical visit. That gives your provider time to review your records and labs, examine your joints, and understand your history before recommending treatment — so you leave with a plan, and honest guidance on what diagnosis and treatment look like from here.
Please arrive about 30 minutes early to complete paperwork — or save time by downloading the new patient packet below, printing it, and bringing the completed forms with you. Please bring:
- Your insurance card(s)
- Pertinent medical records and x-rays
- A list of your current medicines
- A method of payment
- Your referral, if your plan requires one — please confirm it's in hand before your visit
What happens at the visit: (1) we review your records, labs, and imaging; (2) your provider examines you and talks through your symptoms and goals; (3) you leave with a treatment plan and clear next steps.
Co-pays, deductibles, and co-insurance are due in full at the time of your visit.
For referring physicians
Thank you for your referral. Please fax the following to (888) 857-3538:
- A recent physician note stating the reason for consult
- Pertinent laboratory results
- Any previous x-rays, MRIs, or imaging reports
