Cymbalta is a symptom treatment: it strengthens your body’s own pain-dampening system, reducing fibromyalgia and chronic muscle and joint pain. Relief usually begins within 1–2 weeks and builds over 4–6 weeks. It does not repair joints — for osteoarthritis it eases the pain signal, while protecting the joint still depends on strength, weight, and activity.

What it is

Cymbalta (duloxetine) belongs to a class called SNRIs. It adjusts two chemical messengers in the brain and spinal cord — serotonin and norepinephrine — that are involved in both mood and pain control. Raising their levels strengthens the body’s own pain-dampening system, which turns down pain signals before they reach full volume. It is approved for fibromyalgia and for chronic (long-lasting) muscle and joint pain, including chronic low back pain and osteoarthritis pain. Although SNRIs are also used as antidepressants, in fibromyalgia and chronic pain the target is the pain pathway itself.

Before starting

Before starting, we review your other medicines carefully. Duloxetine should not be combined with a small group of medicines (called MAOIs), and combinations with other serotonin-raising medicines need care — this is why we ask for a complete list, including anything bought over the counter. We also ask about liver health and alcohol use, because duloxetine is processed by the liver and heavy alcohol use plus this medicine can strain it. Blood pressure is checked, since duloxetine can nudge blood pressure up in some people — knowing your starting point lets us spot any change at follow-up.

How it’s given

It is a capsule taken by mouth once a day, swallowed whole — do not chew, crush, or open it, because the coating controls how the medicine is released. Treatment often starts at a lower dose for the first week, then moves to the standard daily dose. It can be taken with or without food; taking it with food helps if it makes you queasy at first. Do not stop it suddenly after regular use: abrupt stopping can cause dizziness, electric-shock-like sensations, irritability, and nausea, so we taper gradually when stopping.

What to expect

Pain relief usually begins within 1–2 weeks and builds over 4–6 weeks; sleep and stamina often improve alongside. The most common side effects are nausea in the first days, dry mouth, constipation, reduced appetite, increased sweating, and either sleepiness or trouble sleeping — most settle as your body adjusts. Call us if nausea is severe, if you notice unusual bruising or bleeding (this medicine can slightly affect how platelets work, mainly relevant if you also take blood thinners or daily anti-inflammatories), or if your mood worsens noticeably.

Everyday guidance

Take duloxetine at the same time each day. If you miss a dose, take it when you remember that day; if it is the next day, skip it and carry on — never take two at once. Keep taking it even once pain improves: it maintains the pain-dampening effect only while taken, and stopping suddenly causes the withdrawal-type symptoms described above. Bring your full medicine list to each visit so combinations stay safe. Duloxetine works best combined with movement, sleep habits, and pacing strategies we will discuss. Questions: call (361) 400-7700.

Common worries

Isn’t this an antidepressant? I’m not depressed.

It belongs to the SNRI family, which is used for both mood and pain — because the same messengers control both. For fibromyalgia and chronic pain, the dose targets the pain pathway in the spinal cord. Being prescribed it says nothing about anyone thinking your pain is ‘in your head’.

Can I stop it once I feel better?

Not suddenly — abrupt stopping causes dizziness, irritability, nausea, and electric-shock sensations. When it is time to stop, we taper slowly. Call us first and we will plan it.

Why do you need my full medicine list?

Duloxetine affects serotonin, and combining it carelessly with other serotonin-raising medicines can cause problems. A complete list — including over-the-counter products — is what keeps the combination safe.
