Ilumya is a skin treatment: it calms the signal that drives psoriasis plaques, so skin clears gradually — many people see clear improvement by the second or third dose, judged around 3–4 months. It does not cure psoriasis; the benefit is maintained only by keeping the every-12-week appointments, even when your skin looks completely normal.

What it is

Ilumya (tildrakizumab) is a biologic medicine that blocks interleukin-23 (IL-23) — a protein that helps drive the fast skin growth and inflammation of plaque psoriasis. It is used for adults with moderate-to-severe plaque psoriasis who are candidates for systemic (whole-body) treatment or light therapy. Ilumya is a maintenance treatment: given on a regular schedule, it keeps the signal that produces plaques turned down, helping skin clear and stay clear.

Before starting

Before starting, we screen for tuberculosis (TB) with a blood test. That check matters because immune-calming medicines can allow a hidden, inactive TB infection to become active; if a hidden infection is found, it is treated first so psoriasis treatment can proceed safely. Ilumya is not started while you have an active infection. Live vaccines should be avoided during treatment — if vaccinations are due, completing them before your first dose lets your immune system respond fully. Tell us about any history of frequent infections so we can plan monitoring with you.

How it’s given

It is an injection under the skin given in our office by a member of the clinical team — Ilumya is not self-injected at home. Two starter shots are given a month apart (weeks 0 and 4); after that, one shot every 12 weeks is all that is needed. That means just four short office visits a year for most patients. Each visit is brief, and you can return to your normal activities straight afterwards.

What to expect

Skin clearing builds gradually: many people see clear improvement by the time of the second or third dose, and results are generally judged around 3–4 months. The most common side effects are cold symptoms and other upper respiratory infections, diarrhoea, and brief soreness or redness where the injection was given. Because Ilumya lowers one part of immune defence, infections may be somewhat more likely — call us promptly for fever, a cough that will not settle, or any infection that is not improving. Keep your 12-week appointments even when your skin looks completely clear: staying on schedule is what keeps it that way.

Everyday guidance

Because doses are given in our office, there is nothing to store or inject at home — we simply ask that you keep the appointments on your calendar, and call ahead if you need to move one, so the 12-week rhythm is not lost. If you are ill with an infection near a dose date, call us first; we may briefly delay the shot until you recover. Tell other clinicians you are on an immune-calming biologic, and check with us about vaccines or planned surgery. If you are planning a pregnancy, discuss timing with your provider. Our number is (361) 400-7700.

Common worries

Why do I have to come to the office for it?

Ilumya is approved to be given by a healthcare professional, so doses are given in our office. The upside: only four short visits a year, no injections or storage at home, and the team checks your skin and how you are doing at each visit.

Will it weaken my immune system?

It turns down one signal (IL-23) rather than the whole immune system. Infections — usually colds — are somewhat more likely, so we screen for TB first and ask you to call about fevers or lingering infections.

What if I miss an appointment?

Call us and we will rebook promptly. Staying close to the 12-week rhythm is what keeps skin clear, but one rescheduled visit is easily fixed.
